Patients with venous thromboembolism (VTE) are frequently diagnosed to have inherited thrombophilia. The identification of inherited thrombophilias is a crucial step in the management of at risk patients. In most cases the genetic defect is a mutation in one of the five most important coagulation genes: factor V Leiden (FVL), prothrombin G20210A, protein C and protein S deficiencies, and hyperhomocysteinemia. In the last two decades the genetic diagnostics has achieved great precision due to molecular biology techniques that have led to the discovery of mutations leading to the most common thrombophilia disorders. These techniques have greatly facilitated the identification of new gene defects with clinical impact.
