Immunotherapy with autologous dendritic cells (DCs) transfected with Hep3B HLA DRB1*0301/A*0201-restricted TNF-alpha mutant has been shown to be effective in HLA-matched patients with hepatocellular carcinoma. This phase II trial explored the safety and efficacy of this therapy in HLA haplotype-mismatched patients with advanced hepatocellular carcinoma. Patients with HLA haplotype-mismatched advanced hepatocellular carcinoma received Hep3B-DC therapy every 4 weeks, which consisted of two administrations of 5 x 10(6) Hep3B dendritic cells transfected with the mutant TNF-alpha gene loaded with autologous tumor lysate with or without autologous peripheral blood mononuclear cells. Fifty-seven patients were enrolled in the trial. The median dose per patient was four vaccinations (interquartile range, 2-6.2). Grade 1-2 treatment-related toxicity was noted in 32% of patients, while grade 3-4 toxicities were observed in 6% of patients. The overall objective response was 33%, with a median progression free survival of 9 months. The immunotherapy was well tolerated and a 33% objective response rate was observed in untreated HLA haplotype-mismatched patients with advanced hepatocellular carcinoma.Q:
