A previous study showed that cysteinyl-leukotrienes were involved in both histamine- and nonhistamine-mediated phases in compound 48/80-induced immediate reactions in humans. It is possible, however, that other mediators such as thromboxane are also involved in the nonhistamine-mediate phase. As mast cells contribute to the production of these thromboxane, a possible contribution of mast cells to compound 48/80-induced immediate reactions was investigated by treating compound 48/80-induced immediate reactions with cysteinyl-leukotriene hydrolase and histamine methyltransferase, together with ketotifen, to completely inhibit cysteinyl-leukotrienes and histamine. Ketotifen inhibited compound 48/80-induced immediate reactions without any change in the threshold of histamine response after compound 48/80 challenge. Conversely, combined treatment with both the enzymes completely inhibited compound 48/80-induced immediate reactions but not histamine threshold shifts. When leukotriene D4, a cysteinyl-leukotrienes, was administered by aerosol or by intradermal injection in ketotifen-treated subjects, compound 48/80-induced immediate reactions and threshold shifts were not significantly inhibited in association with the aerosolization or intradermal injection. The results suggest that neither mast cells nor cysteinyl-leukotrienes, a potent mediator of allergy, alone, but that other mediators together or through the histamine-independent pathway, play a role in the nonhistamine phase of compound 48/80-induced immediate reactions.[The “Museum of Surgical Instruments” – a practical manual for the development of surgical education.
